Derrick Matthew Dale, Age 42, dies on Thursday at his home in Pulaski.

He was born in Pulaski.

Visitation will begin on Sunday from 1pm until service at 3pm at Carr and Erwin Funeral Home.

Burial will follow at a later date at Giles Memory Gardens.

Survivors include his mother, Eileen Davis Dale of Pulaski, TN. his wife Lisa Dale if Pulaski, son, David Dale of Pulaski, one daughter, Cori Dale of Pulaski, a brother, Chris Dale of Lynnville, and sister, Sherry Adams of Pulaski, TN. and grandson, Maddox Dale.

Memorial contributions may be made to the Clifford Adams Memorial Scholarship Fund.
